What external-validity concerns survive the clean design?

Internally this is about as good as it gets for a social program. But: it's one state, a specific low-income population, the first year only, and many outcomes are self-reported. Effects on, say, longer-term physical health markers were noisier. Strong identification doesn't automatically mean the magnitudes transfer to other settings.

Right — and short follow-up matters for outcomes that take years to move (chronic disease). The financial-strain and mental-health effects are the most robust early findings; physical-health claims need the longer follow-ups.
